Choosing a pediatrician is one of the most important decisions you make for your child. At Kennesaw Pediatrics, we combine advanced medical care with a personal, hometown touch. We are honored to partner with you in your child’s health and development.
To make your first visit seamless, please follow these four steps:
Step 1: Schedule Your Visit
Call our dedicated appointment line at (770) 429-1005. If you are an expecting parent, ask about our “Mommy Tours” or “Prenatal Consults.”
Step 2: Complete Your Online Forms
Save time by completing our secure digital registration before you arrive.
Step 3: Transfer Medical Records
If moving from another practice, please download our Records Release Form below and send it to your previous pediatrician so we have your child’s history and immunization records (Form 3231) ready.
Step 4: Confirm Insurance
Review our Accepted Insurance List or call your provider to ensure Kennesaw Pediatrics is in-network.

What to Bring to Your First Visit
Ensure a smooth check-in by having the following ready:

Photo ID: For all visiting parents/guardians.

Insurance Card: We must verify coverage at every visit.

Immunization Record: Specifically the Georgia Form 3231 if coming from out of state/practice.

Discharge Paperwork: (For newborns only) Vital health info from the hospital.

Medication List: Any current prescriptions or supplements your child is taking.

A “Comfort Item”: A favorite toy or blanket to help your child feel at home.
